Indian Registry and Status Cards

Status Cards can now be done at the Skidegate Band Council office.

Please bring one piece of Identification which can include:
• Valid Canadian passport
• New plastic certificate of Indian Status card issued after 2008
OR
Two pieces of secondary indentification (one with a digitized photo) which can include:
• Certificate of birth
• Certificate of marriage or divorce
• Provincial health card

• Provincial identification card
• Dirver’s license
• Employee ID with digitized photo
• Student ID with digitized photo
• Firearms license
• Current CIS card ( CIS cannot be expired by more than 6 months)

ID must be intact and readable. Expired ID, SIN cards, photocopied ID or Certified copied ID will not be accepted.

Your previously issued status card (even if expired) must be returned to the issuing officer upon receipt of your replacement/renewal status card.

For children (15 and under)
For clients 15 years of age and under, an application for Certificate of Indian Status (CIS) must be accompanied by prent or legal guardian.
One piece of primary ID which can include:
• Valid Canadian passport
• New plastic certificate of Indian Status card issued after 2008

OR

Two pieces of secondary indentification (one with a digitized photo) which can include:
• Certificate of birth
• Provincial health card
• Provincial identification card
• Student ID with digitized photo
• Firearms license
• Current CIS card ( CIS cannot be expired by more than 6 months)

If applicable a copy of the legal guardianship order naming the guardian, if the legal guardian is applying on behalf of the child

Child
One piece of ID which can include:
• Valid Canadian passport
• New plastic certificate of Indian Status card issued after 2008
• Certificate of birth
• Provincial health card
• Provincial identification card
• Current CIS card ( CIS cannot be expired by more than 6 months)

For children between 12 and 15 years old applying for a renewal or replacement only, please follow identification requirements for adults (First CIS must be applied by parent or guardians).

Photos
If a photo is required; please allow for more time to do them. Skidegate members will only be processed at this time. A notice will be put in the Skidegate Community Newsletter if this changes

OF SPECIAL NOTE:

For people who have applications in under the ‘adoption unit’ in Ottawa, pleae call them or mail in your new address if you have moved. I have spoken to a representative on a file and they close them if they are wanting to correspond with the applicants and they recieve ni reply. If you rapplication was with Alrita make sure your contact address is yours and not Alrita’s office in Prince Rupert. The phone number to call is 1.888.953.6241 and ask what the status of your applications is. You should have received an acknowledgment of your application and they would have given you a file number.

 

You will have to leave a message with that file number. If you do not have a file number you can leave your full name and birthdate and they can readily check on the status of your file. The ‘adoption unit’ would be corresponding with the adopting parent(s).
